Our Mission

The transition program's mission is to assist young people ages 16 to 21 with severe emotional or behavioral difficulties in their transition to adulthood.

Program Description

This transition is done by expanding community capacity to serve the targeted population by:

Resource Case Management

Vocational training

teaches and prepares youth in obtaining and maintaining work.

Actualizing activities include

  • assisting youth in completing job applications appropriately
  • teaching them about time management and good work and study habits
  • providing a safe environment for mock interview processes so that they can grasp and understand the complexity of the job interview
  • developing resumes, cover letters and references, as well as, opportunities to network in their field and communities

Educational training provides

  • support and guidance in completing GED or high school in preparation for further academic and vocational training at a post-secondary facility,
  • assistance in deciding on a career, college applicantion and registration, and financial aid forms such as the standard FAFSA form
  • advocacy for appropriate accommodations and modifications

Life skills training

provides

  • instruction in personal management and general social skills
  • various teaching techniques which include: discussion, demonstration, modeling, feedback and reinforcement

Housing support provides young people with

  • coaching for independent living
  • experience in managing budgets and unexpected expensesopportunities to understand and prevent eviction processes
  • techniques to locate appropriate housing when constrained by income and other criteria

Mental health services/substance abuse services

provides young people with
  • resources for support when needed via comprehensive assessment and interagency staffing

Family support services

provides young people with

  • interpersonal skills to build their personal, cultural and familial support systems
  • know-how in promoting their program retention (follow-through) and in maximizing healthy outcomess

Wellness Recovery Action Plan (WRAP)

provides young people with

 

  • a system of monitoring, reducing and eliminating uncomfortable or dangeorus physical symptoms and emotional feelings.
