| Comprehensive assessment and interagency staffing | This assessment will include complete assessment and interagency staffing. |
Service Plan |
Out of this staffing the youth, family and case worker complete a comprehensive service plan that includes transition goals when appropriate. This plan is also included in the IEP process for youth when appropriate. |
| Resource Case Management | All necessary resources for this youth and vamily will be identified in this plan. Resources may include life skill training,e ducation and vocational training,s ocial and recreational support, housing supports, mental ehalth services, substance abuse services, developmental disability services, case management and linkage to other associated services. |
| Family Support Services | Both youth and family are suported and encouraged to increase family, social, cultural and other support systems to help promote retention and maximize a healthy outcome. |
| Reporting | Monthly reports document the progress on each participant. |
